Application Instructions and Forms

Alaska statutes prohibit unlicensed practice. Specifically, AS 08.68.360 states that the practice of professional or practical nursing for compensation by a person who is not licensed, or whose license is suspended, or revoked, or expired, is declared to be inimical to the public welfare and to constitute a public nuisance.

REGISTERED OR PRACTICAL NURSE

APPLICATION BY EXAMINATION

Application by examination is applicable if you have completed an approved nursing program and have not yet taken/passed the NCLEX (or if your out-of-state license is not current).

Request Fingerprint Cards
Please click the above link to submit a request for a fingerprint card through the mail. You can also use form FD-258 available in many U.S. cities to have your fingerprints taken. One complete fingerprint card is now required.

APPLICATION BY ENDORSEMENT

Application by endorsement is applicable for individuals currently licensed in another U.S. state or territory and some Candian nurses licensed before 1992.

ADVANCED NURSE PRACTITIONER OR CERTIFIED NURSE ANESTHESIST

Individuals must hold a current RN license to be eligible for an ANP or RNA authorization. Applicants must meet educational and national certification requirements. See Article 4 of 12 AAC 44. for Advanced Nurse Practitioners and Article 5 of 12 AAC 44. for Registered Nurse Anesthetists.

GRADUATES OF FOREIGN NURSING SCHOOLS

Graduates of a foreign nursing school (except Canada) must be certified through the Commission on Graduates of Foreign Nursing Schools (CGFNS). More information about requirements for foreign graduates and about CGFNS please click here.
 

REINSTATEMENT INSTRUCTIONS AND FORMS

Licenses that have lapsed more than one year may be reinstated by completing the application for reinstatement, paying the license and penalty fee, providing proof of fulfillment of continuing competency requirements and submitting to a State of Alaska and FBI background check.

If the applicant has not worked in the preceding five years, the applicant must submit proof of completion of a course of study approved by the board (12 AAC 44.317 (b)(5).

A license that has lapsed more than five years will be reinstated by meeting the requirements listed above and AS 08.68.251(b). (See 12 AAC 44.l317 and AS 08.68.251(b).

Before a license can be reinstated, registered and practical nurse licensees must complete two of the following methods for maintaining continuing competency (Article 6 of 12 AAC).

   (a) 30 contact hours of continuing education prescribed under 12 AAC 44.610
   (b) 30 hours of participation in professional activities prescribed under 12 AAC 44.620
   (c) 320 hours of employment prescribed under 12 AAC 44.630
