RN/LPN Applications

Alaska statutes prohibit unlicensed practice. Specifically, AS 08.68.360 states that the practice of professional or practical nursing for compensation by a person who is not licensed, or whose license is suspended, or revoked, or expired, is declared to be inimical to the public welfare and to constitute a public nuisance.

REGISTERED OR PRACTICAL NURSE

APPLICATION BY EXAMINATION

Application by examination is applicable if you have completed an approved nursing program and have not yet taken/passed the NCLEX (or if your out-of-state license is notcurrent).

Request Fingerprint Cards
The fingerprint card FD 258 is available at many locations. You do not have to obtain the card from the Board of Nursing. Go to a business or law enforcement office that provides fingerprinting services and ask if they have form FD 258. No other card is acceptable. However, if you need us to send you a fingerprint card, click the above link to request it. The card will be sent by mail. Applicants must use the card provided by the Board of Nursing or form FD-258 (available in many U.S. cities) to have your fingerprints taken. Submit the completed fingerprint card with your application. All applications will be considered incomplete until a completed card is submitted.

APPLICATION BY ENDORSEMENT

Application by endorsementis applicable for individuals currently licensed in another U.S. state or territory and some Canadian nurses licensed before 1992.

GRADUATES OF FOREIGN NURSING SCHOOLS

Graduates of a foreign nursing school (except Canada) must be certified through the Commission on Graduates
