Dentists and Dental Hygienists who have been engaged in active duty military service in armed forces of the United States during the concluding licensing period for which continuing education is required may be exempt from meeting continuing education required for license renewal. Refer to AS 08.01.100(f).

Other application forms may be mailed to you or picked up at a division office. Completed applications must be signed and submitted to the division with the appropriate fees.
