Renewal Applications

Renewal forms for the July 1, 2012 – June 30, 2014 licensing period will be mailed and be available online approximately 30 days before the June 30, 2012 expiration date of your license.

Please check on the Professional License Search to see if your address of record is current.

If your address is not current, fill out the Change of Address form and mail it to the Division at the address listed on the form.

If you wish to renew your license through June 30, 2012, contact the Division at (907) 465-2551.
